1.Click on the Report button on the tool bar
2.Select the destination for your report, Click OK
3.Using the <, > and <> buttons, select the fields you want to include in your report
4.Click OK
5.Click Yes if you would like to save the report as a template so you can use it again in the future.
6.Enter a Name for the template.
7.Enter a Description of the template
8.Click OK to confirmation
